Community Services Foundation understands that not every person requires full-time services. Programs are available for people with disabilities who reside in the community.

Home Based Support Services
Home based support services allow those eligible to remain living with their families or in their own homes. The services are tailored to each individual.

Community Early Intervention
Community Early Intervention offers home-based or center-based developmental therapy for children under the age of 3.

Physical, Occupational and Speech Therapy
Services are provided by the Phoenix Center in Tinley Park. Traditional therapies as well as Pediatric Intensive Suit Therapy are available for children or adults with cerebral palsy or other neurological disorders.
The Phoenix Center also offers school evaluations and therapy in the school setting.
Additionally, the center offers functional training with prosthetic devices and therapy for people recovering from catastrophic injuries.

Bogard Program
The Bogard program is based in Tinley Park and was created in response to the 1987 class action lawsuit filed on behalf of approximately 3,000 people in the state of Illinois with developmental disabilities. The purpose of the program is to provide services to individuals with developmental disabilities who are 18 years of age or older or who have resided in long-term care facilities on or after March 23, 1986.
The program serves approximately 25 people who reside in area nursing homes. The goal of the program is to increase the participants' independence and quality of life. Each person is paired with a part-time staff member who accompanies them on local community outings.
